Job Description
Roles & Responsibilities
- Conduct comprehensive health assessments to create personalized care plans that address the unique needs of each client.
- Administer medications and treatments as prescribed, ensuring adherence to the specific medical protocols and safety standards.
- Monitor vital signs and health changes, documenting observations meticulously to inform ongoing care decisions.
- Provide emotional support and education to clients and their families, empowering them to manage their health conditions effectively.
Desired Candidate Profile
Bachelor's degree in Nursing (BSc) or equivalent; a higher qualification is preferred for advanced roles.
Active nursing license in the relevant jurisdiction, with a preference for candidates holding additional certifications in home care.
Minimum of 2 years of nursing experience, with at least 1 year specifically in home care or community health settings.
Experience with geriatric care, chronic disease management, and palliative care is highly desirable.
