Job Description
Roles & Responsibilities
We are seeking a compassionate and dedicated Social Worker to support individuals and families in need. The successful candidate will assess client needs, develop care plans, provide counseling, and connect clients with community resources to promote well-being and positive outcomes.
Key Responsibilities:
Conduct client assessments to understand their needs, strengths, and challenges.
Develop individualized care plans in collaboration with clients and their families.
Provide counseling and emotional support to clients facing various social, emotional, or behavioral issues.
Advocate for clients to access necessary community resources, including healthcare, housing, and financial assistance.
Maintain accurate, confidential case records and documentation.
Monitor and evaluate client progress and modify care plans as needed.
Collaborate with multidisciplinary teams, including healthcare professionals, educators, and legal authorities.
Stay informed about legal, social, and community resources and services.
Promote awareness of social issues and contribute to community outreach programs.
Qualifications:
Bachelor s Degree in Social Work (BSW) or related field; Master s Degree in Social Work (MSW) preferred.
Relevant licensing or certification as per regional requirements.
Proven experience in social work or related roles.
Strong communication, interpersonal, and problem-solving skills.
Ability to handle sensitive situations with empathy and professionalism.
Knowledge of social service systems, legal regulations, and community resources.
