· Aids patients in improving their basic motor functions and reasoning abilities, help patient develop, recover, or maintain daily living and work skills.

· Incorporates physical exercises to increase patients’ strength and dexterity.

· Uses programs to help patients improve decision-making, abstract reasoning, problem solving, perceptual skills, memory, sequencing and coordination.

· Assist patients with permanent disabilities in the use of adaptive equipment, such as wheelchairs and orthotics and train them or the appropriate staff for proper care as directed and assure continuation of therapeutic plan.

· Upon referral, evaluates patient to determine problems and goals of each patient in cooperation with physician.

· Collaborate, support and administer medically prescribed occupational therapy treatments/plan of care to treat patients by establishing, developing and/or maintaining their ability to complete daily functions and tasks.

· Performs periodic re-evaluation of the patient as necessary and makes adjustments in the patient’s treatment program.

· Responsible for reporting on the condition of patient/client to the appropriate supervisor and other staff members.

· Thoroughly documents and maintain clinical and administrative records regarding patient limitations/interventions, treatments, responses and progress to administered therapy as well as discharge summaries. (Evaluation, SOAP Notes - daily treatment record, physician update, patient care plan updates).

· Participate in multidisciplinary rounds/meetings as needed.
